If your report shows a high ESR, you probably want to know two things: is it dangerous, and what caused it. This guide explains what the number measures and which limits fit your age and sex. It also shows how ESR fits with CRP, haemoglobin and fibrinogen, and when a high value needs a doctor quickly.

What does a high ESR mean?

A high ESR means your red blood cells sank faster than normal in a tube over one hour, which usually signals inflammation somewhere in the body. It is a nonspecific marker. It tells you that something is going on, but not where or what. That is why it is never read alone.

In the lab, blood stands in a thin upright tube. After one hour, the lab measures how many millimetres of clear plasma sit above the settled cells. The result is given in mm/h. Inflammation raises proteins in the blood such as fibrinogen. These proteins make red cells clump, so they fall faster.

MedlinePlus lists many conditions linked with a raised ESR. They include infections, rheumatoid arthritis and other autoimmune diseases, inflammatory bowel disease, kidney disease, heart disease and some cancers. Two conditions stand out: giant cell arteritis and polymyalgia rheumatica. For these, ESR helps doctors diagnose and monitor the disease.

What is the normal range for ESR?

For many adults an ESR up to 20 mm/h is within range, but the upper limit depends on age and sex. Men under 50 usually stay at 15 mm/h or lower. Women over 50 can reach 30 mm/h and still be normal. The range printed on your own report is the one your lab uses.

You may see an age-based formula for the upper limit online. The sources behind this article do not state one, so we do not give it here. If your report shows 25 mm/h at age 65, first compare it with the range your lab printed.

Why is ESR higher in women and older people?

ESR rises naturally with age and is higher in women, so a value that is raised for a young man can be normal for an older woman. MedlinePlus states that normal ESR varies with age and sex. It adds that pregnancy and exercise can affect the result.

Anaemia is another common reason. StatPearls lists anaemia among the conditions that push ESR up. Our knowledge base notes the same: a moderate rise is often due to age, anaemia or pregnancy. So if your haemoglobin is low, a mild ESR rise may partly reflect the anaemia, not a new inflammation. Advanced kidney disease can raise ESR too.

ESR and CRP both high: what does it mean?

If ESR and CRP are both high, an active inflammation is likely, and it is worth finding the source with your doctor. CRP responds faster than ESR. MedlinePlus notes that CRP is often done together with ESR for this reason: together they give more information.

Think of CRP as a fast signal and ESR as a slow one. CRP rises and falls with how much inflammation is present now. ESR responds more slowly, so it can lag behind after an infection settles. Patterns: ESR with other markers

The single ESR number says little. Most of the meaning sits in the pattern with other markers from the same report. These are the combinations Blood Lab 360 looks at.

  • High ESR and high CRP. Active inflammation is likely. Our hs-CRP band for raised inflammation starts above 3 mg/L. Find the source first; do not only repeat the test.
  • High ESR and normal CRP. Often the slow tail of a recent infection. It can also reflect factors that raise ESR without active inflammation: age, female sex, pregnancy, anaemia or kidney disease.
  • High ESR and low haemoglobin. WHO (2024) defines anaemia as haemoglobin below 120 g/L (12 g/dL) in women and below 130 g/L (13 g/dL) in men. Anaemia can raise ESR on its own. Clear up the anaemia first; the full blood count guide explains the next steps.
  • High ESR and high fibrinogen. Our fibrinogen band is raised above 4 g/L (400 mg/dL). Fibrinogen is one of the proteins that drives ESR up, so both rising together fits an inflammatory state.
  • High ESR and abnormal white cells. Read ESR with the white cell count and its differential. A shift there helps show what kind of process is going on.
  • ESR above 100 mm/h. StatPearls links this with serious disease, such as infection, multiple myeloma, giant cell arteritis or polymyalgia rheumatica. This needs a doctor soon, not a repeat in two months.

Is an ESR of 40 dangerous?

An ESR of 40 mm/h is above range for most adults and falls in our raised band, but on its own it is not an emergency. What matters is your age, sex, CRP, haemoglobin and symptoms. In an older woman with anaemia it can be a mild finding. In a younger man with fever or weight loss, it needs a prompt check.

If you feel well, ask your GP to repeat ESR with CRP and a full blood count once any recent infection has cleared. If it stays the same or climbs, the next step is a search for the cause. See a doctor quickly if a high ESR comes with fever, night sweats or weight loss you cannot explain. The same applies to a new headache or changes in vision, especially over 50, and to new pain and stiffness in the shoulders or hips. These can point to giant cell arteritis or polymyalgia rheumatica, where ESR is used. An ESR above 100 mm/h also needs a doctor within days. Sudden loss of vision is an emergency: seek care the same day.
