ALP appears on almost every liver panel, yet it is not only a liver test. This guide explains what a high or low result means and how to find its source. It also shows which combinations with other markers deserve a closer look.
What does high alkaline phosphatase mean?
High alkaline phosphatase means your blood holds more of an enzyme made mainly in the liver, the bile ducts and bone. The number alone does not tell you which of these it came from. That is why ALP is always read with other markers, above all GGT.
In the liver, ALP sits in the cells that line the small bile ducts. When bile does not flow freely, these cells release more of it. Doctors call this slowed bile flow cholestasis. In bone, ALP comes from the cells that build new bone. Anything that speeds up bone building raises it: growth, a healing fracture or softened bone.
ALP stays in the blood for about seven days (StatPearls). So when the cause goes away, the value falls over weeks, not overnight.
What is the normal range for alkaline phosphatase?
For adults, Blood Lab 360 treats ALP up to 120 U/L as within range, 120–150 U/L as mildly raised and above 150 U/L as raised. These are common laboratory reference intervals. ALP methods differ, so the limits printed on your own report come first.

Why is ALP higher in teenagers and in pregnancy?
Children, teenagers and pregnant women often have ALP above the adult range, and this is expected. Growing bones release bone ALP, so levels are highest during childhood and puberty (StatPearls). In the late third trimester, the placenta adds its own ALP to the blood.
A value that would count as raised at 45 can be ordinary at 14. Our ALP bands are adult bands. If you are a teenager or pregnant, read a high result against the age or pregnancy ranges from your lab, together with your doctor.
One more everyday effect: in people with blood group O or B, a fatty meal can raise ALP a little, from the gut (StatPearls). This is one reason labs ask you to fast before the test. Our guide on preparing for a blood draw covers the rest.
How do ALP and GGT tell liver from bone?
GGT is the quickest way to find the source of a high ALP. GGT is found in the liver and bile ducts but not in bone. If ALP and GGT rise together, a liver or bile duct source is more likely. If ALP is high and GGT is normal, bone becomes the more likely source (StatPearls, BSG 2018).
A trap we often see on reports: ALP is flagged, but GGT was never measured. Without GGT the question stays open. Asking for GGT at the re-test is often the most useful next step. Our article on ALT, AST and GGT explains GGT in more detail.

Which combinations with GGT, bilirubin, ALT, calcium and vitamin D matter?
The pattern across several markers says far more than ALP alone. These are the combinations Blood Lab 360 looks for:
- ALP high, GGT high, ALT normal or mildly raised. This is a cholestatic pattern: bile flow is slowed. Causes include a blockage in the bile ducts, some medicines and autoimmune bile duct diseases. BSG 2018 suggests an abdominal ultrasound and an antimitochondrial antibody test to look for primary biliary cholangitis.
- ALP, GGT and bilirubin all high. Bile may not be draining properly. With yellow eyes or pale stools, this needs a doctor within days. Read more in high bilirubin.
- ALT much higher than ALP. This points to strain on liver cells rather than on bile flow. Fatty liver is a common reason; see fatty liver and FIB-4.
- ALP high, GGT normal, vitamin D low. This points to bone. Low vitamin D can soften bone (osteomalacia), and bone-building cells then work harder. See our vitamin D guide.
- ALP high, GGT normal, calcium high. This can fit overactive parathyroid glands, which speed up bone turnover. A doctor may add PTH.
- ALP high, GGT normal, with bone pain. Paget's disease of bone, a healing fracture or, less often, cancer that has spread to bone are possible. This needs a doctor, not waiting.
What does low alkaline phosphatase mean?
Low alkaline phosphatase is uncommon and usually not urgent. The enzyme needs zinc to work, so a zinc deficiency can lower it. MedlinePlus and StatPearls also list protein deficiency or malnutrition, thyroid disease, pernicious anaemia (a form of B12 deficiency) and Wilson disease.
A rare inherited condition, hypophosphatasia, keeps ALP low on every test. It affects bones and teeth. It is worth raising with a doctor when ALP stays low on repeat tests and you have fractures or dental problems.
Our bands do not set a lower ALP limit, because the lower limit varies with the lab method. If your report marks ALP as low, repeat the test and look at zinc, TSH and B12 first.
See a doctor quickly if a raised ALP comes with yellow skin or eyes, dark urine, pale or clay-coloured stools, itching, pain in the upper right belly or fever. The same applies when ALP, GGT and bilirubin all rise together, or when you have new bone pain or a fracture after a minor fall. One mildly raised ALP without symptoms is rarely an emergency. The pattern and the trend over time matter more.
