MPV is one of the less explained numbers in a complete blood count. This guide covers what high or low MPV means, which patterns with the platelet count matter, and why labs disagree on the range. It builds on our platelet count guide.

What does MPV mean on a blood test?

MPV stands for mean platelet volume, the average size of the platelets in your sample. MedlinePlus describes the MPV blood test as a test that measures the size of your platelets. The analyzer reports it in femtoliters (fL) together with the platelet count in your complete blood count.

Platelets are small cell fragments made in the bone marrow. They stop bleeding by clumping at a damaged vessel. Young platelets fresh from the marrow tend to be larger. This is why platelet size can hint at how fast the marrow is replacing them.

On a Slovenian report, MPV appears as "K-MPV". The K- prefix means the test was run on whole blood (kri). You will often see PDW (platelet distribution width) and RDW nearby. RDW describes red cell size, not platelets. Our RDW guide covers it.

What is a normal MPV level?

A normal MPV is usually about 8 to 12 fL, according to a 2019 review in the Cleveland Clinic Journal of Medicine. The same review stresses that MPV varies by instrument. Each lab therefore sets its own reference range.

One example is the Golnik clinic lab list from 2025. It gives 9.1–12.1 fL for adults over 18. Another lab with a different analyzer may print a different range. The range on your report is the one that applies to your result.

What does a high MPV mean?

A high MPV means your platelets are larger than average. Its meaning depends on the platelet count. When platelets are low and MPV is high, the CCJM review explains that platelets are often being destroyed in the bloodstream. Immune thrombocytopenia (ITP) is the classic example.

The body reacts to the loss. It makes more thrombopoietin, the hormone that drives platelet production. Healthy marrow then releases new, larger platelets, and the average size goes up. A high MPV in this setting suggests the marrow is working.

Rare inherited platelet disorders can also show a high MPV with a low count. The review names Bernard-Soulier syndrome and gray platelet syndrome. These are usually known from childhood.

A high MPV has also been linked with worse outcomes in heart disease and with type 2 diabetes in research studies. The review calls the role of MPV in predicting outcomes investigational. A high MPV on a routine count does not diagnose heart disease.

What does a low MPV mean?

A low MPV means your platelets are smaller than average. With a low platelet count, the CCJM review explains that it points to a marrow that makes too few platelets. The platelet-making cells (megakaryocytes) are reduced or not working well, so they cannot keep up the total platelet mass.

A rare inherited condition, Wiskott-Aldrich syndrome, shows low platelets with a low MPV. In people with cancer, the review reports that a low MPV has been linked with a poorer outlook. This is a research finding, not a test for cancer.

A low MPV with a normal platelet count and a normal blood count is not described in the source as a sign of disease. Lab variation is a more likely reason.

What do high or low MPV and the platelet count mean together?

MPV only makes sense next to the platelet count. The CCJM review calls the product of MPV and the platelet count the total platelet mass. These are the patterns the review describes, plus what to do when it describes none:

Two more checks help in practice. If platelets, hemoglobin and white cells are all low, more than one cell line is down. That points to the marrow and needs a doctor soon. Why is my MPV high when my platelets are normal?

A high MPV with a normal platelet count is most often a measurement effect, not a disease. The CCJM review states that MPV measurement is subject to significant lab variation. Different analyzers use different methods and give different numbers for the same blood.

Sample age matters too. A 2021 study in the Journal of Medical Biochemistry stored blood samples at three temperatures. MPV stayed fairly stable for about 16 hours. It rose significantly between 20 and 48 hours at all three temperatures. The authors link this to platelet swelling in the tube.

So a sample sent from a remote draw site, or measured the next day, may show a higher MPV. If everything else in your count is normal, a single high MPV rarely needs action. For a general approach to a flagged value, read a value outside the reference range.

See a doctor soon if your platelet count is below the range together with unusual bruising, nosebleeds, bleeding gums or tiny red spots on the skin. The same applies if platelets, hemoglobin and white cells are all low at once. Get urgent care for heavy bleeding that does not stop. An abnormal MPV alone, with a normal count and no symptoms, is not an emergency.
